Abstract: Introduccion. El tratamiento coadyuvante con vitaminas C y E ha sido propuesto como una alternativa para incrementar las tasas de erradicación atribuibles a la terapia triple estándar para Helicobacter pylori, estimada por debajo del 80%. Este estudio tiene por objetivo determinar la efectividad atribuible al tratamiento coadyuvante con vitaminas C y E para H. pylori en una cohorte de pacientes de la Amazonía Peruana tratados con la terapia triple estándar (TTE). Materiales y métodos. Siguiendo un diseño de cohortes se evaluó retrospectivamente una cohorte de 50 pacientes infectados con H. pylori tratado con TTE (Amoxicilina 1g BID, claritromicina 500 mg BID y omeprazol 20mg BID por 14 días) en el Hospital de Tarapoto en julio-diciembre 2016. La mitad fueron expuestos al tratamiento coadyuvante con vitaminas C y E y la otra mitad fue tomada al azar (1:1) entre los no expuestos a la terapia adyuvante. Con los datos de sus historias clínicas y teniendo como variable desenlace la erradicación del H. pylori confirmada por histopatología, se estimó y comparó la efectividad de ambos tratamientos utilizando un modelo regresión general lineal con familia Poisson y link log. Resultados. En el análisis de línea de base se encontraron diferencias significativas entre los expuestos y no expuestos, incluyendo variables como edad (38 ± 11 vs. 36 ± 10 años), sexo masculino (65% vs. 63%), síntomas principales (anorexia, 13% vs. 4%; dolor abdominal, 43% vs. 40%; náuseas, 4% vs. 9%; pirosis, 17% vs. 18%; regurgitación, 43% vs. 40%) y diagnóstico histopatológico basal (gastritis antral, 54% vs. 43%; dolor abdominal, 18% vs. 13%; pangastritis, 27% vs. 43%). Al término del tratamiento se encontró un incrementó significativo en las tasas de erradicación en los expuestos con respecto a los no expuestos de un 9,5% (91% vs. 82%, riesgo relativo [RR]=1,11; Intervalo de confianza [IC] al 95%: 0,92 a 1,36). Conclusiones El tratamiento coadyuvante con vitaminas C y E podría ayudar a incrementar la efectividad atribuible a la TTE para H. pylori en los pacientes de la Amazonía Peruana; sin embargo, para demostrar ello se recomienda realizar un ensayo clínico considerando género como un potencial modificador de efecto.
Introduction. Helping treatment with vitamins C and E has been proposed as an alternative to increase the eradication rates attributable to standard triple therapy for Helicobacter pylori, estimated below 80%. The objective of this study is to determine the effectiveness attributable to auxiliary treatment with vitamins C and E for H. pylori in a group of patients from the Peruvian Amazon treated with standard triple therapy (TTE). Materials and methods. Following a group design, a group of 50 patients infected with H. pylori treated with TTE (Amoxicillin 1g BID, clarithromycin 500 mg BID and omeprazole 20mg BID for 14 days) was evaluated retrospectively at the Tarapoto Hospital from July - December 2016. Half of them were exposed to auxiliary treatment with vitamins C and E and the other half were taken at random (1: 1) among those not exposed to auxiliary therapy. With the data of their clinical histories and having as a variable outcome the H. pylori eradication confirmed by histopathology, the effectiveness of both treatments was estimated and compared using a linear general regression model with Poisson family and link log. Results. In the baseline analysis, significant differences were found between exposed and unexposed subjects, including variables such as age (38 ± 11 vs. 36 ± 10 years), male sex (65% vs. 63%), main symptoms (anorexia, 13% vs. 4%, abdominal pain, 43% vs. 40%, nausea, 4% vs. 9%, heartburn, 17% vs. 18%, regurgitation, 43% vs. 40%) and baseline histopathological diagnosis (gastritis antral, 54% vs. 43%, abdominal pain, 18% vs. 13%, pangastritis, 27% vs. 43%). At the end of the treatment, there was a significant increase in the eradication rates in those exposed with respect to the non-exposed of 9.5% (91% vs. 82%, relative risk [RR] = 1.11; [CI] 95%: 0.92 to 1.36). Conclusions. The auxiliary treatment with vitamins C and E could help to increase the effectiveness attributable to TTE for H. pylori in patients from the Peruvian Amazon; however, to demonstrate this, it is recommended to carry out a clinical trial considering gender as a potential effect modifier.
