Abstract: OBJETIVO: Conocer la utilidad del índice leucoglucémico (ILG) ≥ al punto de corte 982 como factor predictor de complicaciones clínica intrahospitalaria en pacientes con enfermedad cerebrovascular isquémico agudo. MATERIAL Y METODOS: Se realizó un estudio observacional, analítico de cohorte retrospectivo, diseño transversal de pruebas diagnósticas, en la que se evaluó 74 pacientes con diagnóstico de enfermedad cerebrovascular isquémico agudo que acudieron al Hospital II-2 Tarapoto durante el periodo enero 2017 al diciembre 2020. Los instrumentos para la recolección de datos fue una matriz con todas las variables propuestas clínicas y de laboratorio, incluyendo glucemia y leucograma al ingreso, a partir de los cuales se calculó el valor del índice leucoglucémico y se evaluó su valor como marcador pronóstico de complicaciones clínicas y mortalidad, así como su relación con la escala de NIHSS. RESULTADOS: La edad media fue de 64 ± 14.88 años, el 45% fueron hombres, la media del ILG de la muestra fue 1387 ± 760.82. Se presentó complicaciones en el 54.20% a predominio de la infección urinaria (18.9%) y la mortalidad fue del 12.20%. La media de la escala NIHSS 8.91 ± 6.98, a predominio del puntaje moderado (41.9%) con un promedio del ILG 1399.64 La media de la estancia hospitalaria fue 7 ± 1.91 días. En pacientes con complicaciones tuvieron una media del ILG 1613 ± 848.3 y con el punto de corte ILG ≥ 982 la curva ROC mostró un área bajo la curva de 0.693. El ILG relacionado con la mortalidad tuvo una media 2407.34.y un punto de corte de 1860. CONCLUSIONES: El índice leucoglucémico ≥ 982 se relacionó positivamente como factor predictor de las complicaciones evaluados con la escala de NIHSS.
OBJECTIVE: To determine the usefulness of the leukoglycemic index (LGI) ≥ 982 as a predictor of in-hospital clinical complications in patients with acute ischemic cerebrovascular disease. MATERIAL AND METHODS: An observational, analytical, retrospective cohort study was performed with a cross-sectional design of diagnostic tests, in which 74 patients with a diagnosis of acute ischemic cerebrovascular disease who attended the Hospital II-2 Tarapoto during the period from January 2017 to December 2020 were evaluated. The instruments for data collection were a matrix with all the proposed clinical and laboratory variables, including glycemia and leukogram at admission, from which the value of the leukoglycemic index was calculated and its value as a prognostic marker of clinical complications and mortality was evaluated, as well as its relationship with the NIHSS scale. RESULTS: The mean age was 64 ± 14.88 years, 45% were men, the mean LGI of the sample was 1387 ± 760.82. Complications occurred in 54.20%, predominantly urinary tract infection (18.9%) and mortality was 12.20%. The mean NIHSS scale was 8.91 ± 6.98, with a predominance of the moderate score (41.9%) with a mean LGI of 1399.64 The mean hospital stay was 7 ± 1.91 days. Patients with complications had a mean LGI of 1613 ± 848.3 and with the cutoff point LGI ≥ 982 the ROC curve showed an area under the curve of 0.693. The LGI related to mortality had a mean of 2407.34. and a cutoff point of 1860. CONCLUSIONS: The leukoglycemic index ≥ 982 was positively related as a predictor of complications in patients with cerebrovascular disease, evaluated with the NIHSS scale.
