dc.description.abstract | Objetivo. Conocer las características clínicas, epidemiológicas y patrón de resistencia en pacientes con tuberculosis pulmonar drogorresistente en mayores de 15 años atendidos en el hospital II-2 Tarapoto en el periodo 2017-2019. Metodología. Investigación observacional, transversal, descriptiva, retrospectiva, en 61 casos de tuberculosis pulmonar drogorresistente atendidos en el Hospital II-2 de Tarapoto. Resultados. Los pacientes con tuberculosis pulmonar drogorresistente en mayores de 15 años atendidos en el Hospital II-2 de Tarapoto el 73.8% sexo masculino y el 26.2% de sexo femenino, el 54.1% fueron de edad de entre 18 a 37 años, 24.6% de entre 38 a 57 años y el 21.3% de entre 58 a 77 años de edad. Los pacientes drogorresistentes proceden de zonas con mayor densidad poblacional como Tarapoto 32.8%. Los pacientes con tuberculosis pulmonar drogorresistente presentaron sintomatología en su mayoría tos con un 91.8 %, seguido de fiebre con un 90.1%, 80.2% de pérdida de peso, 73.7% de sudoración nocturna y 57.4% hemoptisis. El 13.1% nunca fue tratado y el 86.9% fue antes tratado. El 68.9% presentaron comorbilidades siendo éstas la diabetes mellitus tipo 2, VIH, Insuficiencia renal entre las más frecuentes con impacto desfavorable sobre la recuperación y el 31.1% no presentaron comorbilidades. El 18% de los pacientes tuvieron como estado nutricional bajo peso, 62.3% fueron normo pesos y el 19.7% tuvieron sobre peso. El 52.5% tuvieron baciloscopías negativas, el 14.7% tuvieron 1 + por baciloscopía, el 16.4% tuvieron 2 ++ y 16.4% tuvieron 3+++. El 26.2% tuvieron carga bacilar 1 + por cultivo, el 11.5% tuvieron 2 ++ y 62.3% tuvieron 3+. El 16.4% es resistente solo a la isoniacida, el 11.5% es resistente solo a la rifampicina, el 6.5% es resistente solo al etambutol, el 19.7% es resistente tanto a la izoniacida y a la rifampicina y el 45.9% es resistente a diferentes combinaciones de drogas. Conclusiones: Los pacientes con tuberculosis pulmonar drogorresistentes en su mayoría tiene baciloscopías negativas, mientras que la mayoría tienen 3+ en el cultivo, tienen una alta probabilidad de presentar bajo peso, el 86.9% ya tuvieron un tratamiento previo de tuberculosis, son en su mayoría de sexo masculino, y en edades económicamente activas, las resistencias son a las principales drogas utilizadas de primera línea para esta enfermedad y el 68.9% presenta otras comorbilidades como diabetes, VIH/SIDA e Insuficiencia Renal, y proceden sobre todo de Tarapoto. | es_PE |
dc.description.abstract | Objective. To know the clinical and epidemiological characteristics and pattern of resistance in patients with drug-resistant pulmonary tuberculosis in patients over 15 years of age treated at the II-2 Tarapoto hospital in the period 2017-2019. Methodology. Observational, cross-sectional, descriptive, retrospective investigation in 61 cases of drug-resistant pulmonary tuberculosis treated at Hospital II-2 in Tarapoto. Results. 73.8% male and 26.2% female patients with drug-resistant pulmonary tuberculosis in those over 15 years of age treated at Hospital II-2 of Tarapoto, 54.1% were between 18 and 37 years old, 24.6% were between 38 to 57 years and 21.3% between 58 and 77 years of age. Drug resistant patients come from areas with a higher population density such as Tarapoto 32.8%. Patients with drug-resistant pulmonary tuberculosis presented symptoms, mostly cough with 91.8%, followed by fever with 90.1%, 80.2% weight loss, 73.7% night sweats, and 57.4% hemoptysis. 13.1% were never treated and 86.9% were previously treated. 68.9% presented comorbidities, these being type 2 diabetes mellitus, HIV, and kidney failure among the most frequent with an unfavorable impact on recovery, and 31.1% did not present comorbidities. 18% of the patients had underweight as a nutritional status, 62.3% were normal weights and 19.7% were overweight. 52.5% had negative smears, 14.7% had 1 + by smear microscopy, 16.4% had 2 ++ and 16.4% had 3 +++. 26.2% had 1 + bacillary load per culture, 11.5% had 2 ++ and 62.3% had 3+. 16.4% are resistant only to isoniazid, 11.5% are resistant only to rifampicin, 6.5% are resistant only to ethambutol, 19.7% are resistant to both izoniazid and rifampin and 45.9% are resistant to different combinations of drugs. Conclusions: Most of the patients with drug-resistant pulmonary tuberculosis have negative smears, while the majority have 3+ in the culture, they have a high probability of presenting low weight, 86.9% have already had a previous tuberculosis treatment, they are mostly of male sex, and in economically active ages, resistance is to the main drugs used first line for this disease and 68.9% present other comorbidities such as diabetes, HIV / AIDS and Renal Insufficiency, and they come mainly from Tarapoto. | es_PE |
