Abstract: El carcinoma basocelular (CBC) es el cáncer más frecuente a nivel mundial y representa el 75% a nivel de cáncer de piel no melanoma, es destructivo, pero de crecimiento lento y si no es tratado a tiempo puede causar daño en la funcionalidad y estética. Por lo cual, es importante que sea diagnosticado y tratado de manera precoz. El objetivo del estudio fue identificar las características epidemiológicas, clínicas e histológicas del carcinoma basocelular en el Hospital II-2 de Tarapoto, periodo de enero a diciembre 2019. El estudió fue del tipo no experimental, descriptivo, retrospectivo de corte transversal en 53 pacientes con diagnóstico histopatológico de CBC. La técnica que se utilizó fue la revisión de historias clínicas y el instrumento fue una ficha de recolección de datos validada por expertos. Resultados: Se identificaron 65 lesiones tumorales de CBC en 53 pacientes. El 50,9 % de pacientes fueron de sexo femenino. La edad promedio fue de 72,3 años siendo más frecuente en adultos mayores (69,8%). El 62,3% residía en el área Urbana. El 43,4% de pacientes procedían de la provincia de San Martín. En cuanto a las características clínicas, el tiempo de enfermedad osciló entre los 2 y 48 meses y la media fue de 17,5 meses. El 83,0% de los pacientes presentaron lesión única. En cuanto a la localización anatómica de la lesión el 95,4% se presentó en la cabeza y dentro de ésta el 36,9% se localizó en nariz. El diámetro de la lesión osciló entre 3 y 40 mm y la media fue de 12,0 mm. El 86,2% de las lesiones fue del tipo nodular y dentro de este el nodular-ulcerado representó el 61,5%. El 63,1% fueron lesiones asintomáticas, el 24,6% presentaron sangrado, el 10,8% presentó prurito y el 1,5% fueron dolorosas. El 54,7% de las lesiones no recibió ningún tratamiento previo a la consulta dermatológica. El 89,2% de las lesiones fueron primarias y el 10,8 % correspondieron a lesiones recidivantes. A nivel histopatológico, el subtipo más frecuente fue el nodular (56,9%), seguido del subtipo mixto (12,3%); y, en tercer lugar, el subtipo superficial (6,2%). Conclusiones: No hubo diferencia significativa en relación al sexo, la edad de presentación más frecuente fue en adultos mayores, que procedían del área urbana. El tipo clínico más frecuente fue el nodular. La localización más frecuente fue en áreas foto expuestas, siendo predominante en nariz. La mayoría de lesiones eran únicas, no recidivantes, con un tiempo de enfermedad mayor a 12 meses y no presentaron síntomas acompañantes, la mitad de las lesiones presentaron un diámetro mayor a 10 mm. Predominó el subtipo histopatológico nodular, seguido del subtipo mixto.
Basal cell carcinoma (BCC) is the most common cancer worldwide and represents 75% of non-melanoma skin cancer. It is destructive, but slow growing and if not treated in time can cause damage to functionality and aesthetics, which is why it is important to be diagnosed and treated early. The objective of the research was to determine the epidemiological, clinical and histopathological characteristics of basal cell carcinoma at Hospital II-2 Tarapoto, period from January to December 2019. The study was non-experimental, descriptive, retrospective, cross-sectional with 53 patients with histopathological diagnosis of BCC. The technique used was the data collection guide and the instrument was a data collection form validated by experts. Results: Sixty-five BCC tumor lesions were identified in 53 patients. The 50.9 % of patients were female. The average age was 72.3 years being more frequent in older adults (69.8%). The 62.3% resided in the urban area. The 43.4% of patients came from the province of San Martín. Regarding the clinical characteristics, the time of disease ranged from 2 to 48 months and the mean was 17.5 months. The 83.0% of the patients presented a single lesion. Regarding the anatomical location of the lesion, 95.4% was on the head and within this 36.9% was located on the nose. The diameter of the lesion ranged from 3 to 40 mm and the mean was 12.0 mm. Nodular lesions accounted for 86.2% and nodular-ulcerated lesions accounted for 61.5% of the total number of lesions. The 63.1% were asymptomatic lesions, 24.6% presented bleeding, 10.8% presented pruritus and 1.5% were painful. Of the lesions, 63.1% did not receive any treatment prior to dermatologic consultation. Of the lesions, 89.2% were primary and 10.8% corresponded to recurrent lesions. Histopathologically, the most frequent subtype was nodular (56.9%), followed by the mixed subtype (12.3%) and, in third place, the superficial subtype (6.2%). Conclusions: There was no significant difference in relation to sex, the most frequent age of presentation was in older adults from urban areas. The most frequent clinical type was nodular. The most frequent location was in photoexposed areas, predominantly in the nose. Most lesions were single, non-relapsing, with a disease time greater than 12 months and no accompanying symptoms, half of the lesions had a diameter greater than 10 mm. The nodular histopathologic subtype predominated, followed by the mixed subtype.
